*A Duress password is used by you (instead of the General Password) under an intruder threat to discreetly notify our Central Station staff to immediately dispatch the police. If you prefer, you do not need to enter a Duress Password; but, we strongly suggest you do so. The Duress Password may be up to 20 characters.

Please enter below persons that may be called in case of an alarm situation (including yourself again, if you want to list your cell or numbers not listed above to be called). Each person must be listed below in the order you want us to contact them in the event of an alarm. Normally, persons with a key to your residence and knowledge of the code necessary to key into the alarm panel in your home to silence the alarm siren should be listed first.
When Federal Services Corporation receives an alarm call from your alarm panel, we will make no more than 2 calls to resolve an alarm before dispatching emergency services. The first number called will be to either (1) the phone line located in your residence (listed above) OR (2) to the first contact’s “Priority 1 Phone #” listed below in case you don’t have home phone line. If the line is busy, we will re-try this first call only once. If your community has requested that we contact your gatehouse staff as the second call, we will do so. Otherwise, we will call the “Priority 2 Phone #” for the first contact or if no other phone number is specified for the first contact, then the Contact Name 02 “Priority 1 Phone #” will be called. No further calls will be made by us until directed to do so by either the gatehouse guards or emergency services.
The only purpose for listing more than 2 contact phone numbers below is for a case where emergency services or gatehouse guards need to locate someone else with a key to the premises to deal with the emergency, gain access to the home or provide the panel code necessary to silence an alarm.

If you want a contact to use the same password(s) that you specified above, just leave the Password(s) below blank and that contact must know & use the above specified password(s). Remember, we will not discuss an alarm situation with anyone who we cannot authenticate using the passwords specified on this form. So, make sure all contacts listed below know the password that they must use if they receive a call from us regarding an alarm at your home.
